Requirements
- Minimum Diploma (D3) in health-related fields (Public Health, Nursing, Midwifery, Health Administration) or relevant social science background
- Good understanding of patient rights, informed consent, and confidentiality
- Strong communication skills and ability to explain consent clearly in simple language
- Comfortable interacting with patients from diverse backgrounds
- Basic digital literacy for data entry (tablet, smartphone, web-based forms)
- Careful, ethical, and respectful approach to patient interaction
- Able to work collaboratively with Puskesmas clinical and administrative staff
Job description
- Obtain informed consent from all eligible Puskesmas visitors in accordance with approved ethical and operational protocols
- Provide clear explanations to patients regarding:
- The purpose of informed consent
- How data will be used and protected
- Patients’ rights to refuse participation or withdraw consent at any time
- Ensure all informed consent documentation is fully completed, accurate, and properly recorded
- Conduct standardized patient screening to assess whether patients have:
- Received all required services during their visit
- Missed any recommended clinical, preventive, or follow-up care
- Identify and document care gaps, including missed services, delayed follow-ups, or unmet referrals
- Coordinate with health workers to:
- Inform them of identified care gaps
- Support immediate action or scheduled follow-up when feasible
- Ensure that all required data—including data generated from legacy or existing applications used by the Puskesmas—are reviewed, compiled, and uploaded into the designated monitoring system
- Verify completeness, consistency, and basic data quality prior to upload
- Ensure all collected and relevant data are uploaded within 24 hours of the patient visit or data generation
- Record consent status, care gap findings, and follow-up actions into the designated digital system or dashboard in a timely manner
- Maintain strict patient privacy and comply with ethical standards and data protection regulations
- Contribute to continuous quality improvement by providing regular summaries on informed consent coverage, data timeliness, and care gap trends
KPI
- Informed consent coverage rate among eligible Puskesmas visitors (target: 100%)
- Completeness and accuracy of informed consent documentation
- Proportion of patients successfully screened for care gaps
- Number and percentage of identified care gaps appropriately documented
- Percentage of required data (including legacy application data) uploaded within 24 hours
- Timeliness and completeness of data uploads into the monitoring system
- Zero incidents of confidentiality breaches or ethical violations
- Positive feedback from patients and Puskesmas clinical and administrative staff
